How to enroll
Call (409) 755-3933 or ask at the front desk at your next visit. Payment is due in full at enrollment and your benefits start that day.
Membership terms
- This is not dental insurance. It is a discount membership valid only at DuBois Family Dentistry.
- Cannot be combined with dental insurance or other offers.
- Membership fees are non-refundable and non-transferable.
- Benefits must be used within 12 months of enrollment and do not roll over.
- Specialist services, products, and lab fees may not be included.
- SureSmile clear aligners and orthodontic treatment are excluded from the 20% member discount.
- Payment is due in full at the time of enrollment.
